Football Coaching Camps - Booking form

Important information !

>  Please send your child changed and ready to play, with a packed lunch and plenty of drinks.  

>  Shin pads are essential and we recommend a pair of trainers to change into.  

>  Additional warm clothing is advised, particularly for younger children.

>  Children will only be supervised between 10am-3pm.  

>  There will be an emergency first-aider at all venues.

>  We cannot accept responsibility for any loss of money, or belongings, damage or injury arising from the courses whatsoever.

Course Costs

2-Day course = £22      3-Day course = £33        4-Day course = £44

If you haven’t pre-booked, you can still ‘pay-on-the-day’ however this will cost £12 per day.

How to pay

Cheque - payable to ‘Bradford City FITC’

Phone - payment can be made by card by calling our office on 01247 706850. Please note, there is a £1.00 admin fee for payment via this method.

You can also ‘pay-on-the-day’ by cash

Please do not send cash through the post.
